Free AS9102 Form 3 Excel template

Last reviewed: 17 October 2026

Two files, both free, both with the same fifteen columns. One is blank and ready to fill. The other is a complete worked example for a machined bracket, including the one characteristic that failed and how it was recorded. No account, no email address, nothing to install.

Blank template

Fifteen columns, header frozen, Pass/Fail and Inspection Type as dropdowns, and a part-information sheet. Ten numbered rows to start typing into.

Worked example

A mounting bracket, twelve characteristics: dimensions, a hole callout, position, flatness, perpendicularity, surface finish, a thread. One hole is out of tolerance and carries an NC code and a disposition, so you can see how a failure is recorded.

What is in the blank template

The first worksheet holds the fifteen columns and nothing else, so the file stays machine-readable: one characteristic per row, one header row, no merged cells, no title block above the data. A title block in the middle of a spreadsheet is the single most common reason an automated reader gives up.

ColumnHolds
ItemRow number. Renumber after sorting, or the sequence stops matching the report.
Char. No.The balloon number on the drawing. This is what the reviewer uses to find the feature.
Dimension / FeatureThe callout as drawn: 6.35, a datum, GD&T, a note reference.
Nominal, Tol +, Tol −Three separate columns. A bilateral tolerance repeats the same number in both; an asymmetric one does not.
ActualWhat was measured, in the units of the drawing.
Pass / FailTwo values only. Blank is treated as not yet inspected.
Inspection Date, Inspector, Gage IDWho measured it, with what, on what date. The gage ID is the field most often left empty.
NC Code, DispositionOnly on a failed row. A fail with no NC and no disposition reads as an unfinished report.
Inspection TypeFAI, IPQC, Final or Receiving — so a mixed sheet still says which rows belong to the first article.
Source FileDrawing number, revision and sheet, so a row can be traced back without guessing.

What the worked example shows

Numbers alone do not teach a format; a completed row does. The example uses a plausible part and shows four things a blank template cannot:

Why these fifteen columns

The columns are the fields a Form 3 reviewer needs in order to check the report without opening the drawing. They are not a transcription of the standard’s layout — the standard defines what Form 3 must contain, not how it must be arranged in a spreadsheet. What matters is that every characteristic has a row, every row has a nominal, a tolerance, a result and a verdict, and every failure has a record.

If you already have the data

Filling forty rows by hand from a CMM export or a paper form is the part nobody wants. The converter reads an existing Form 3 and fills these fifteen columns for you, in the browser, with nothing uploaded. The files on this page are the format it writes, so anything you build here drops straight back in.

Retyping these columns by hand? The converter reads an AS9102 Form 3 and fills all 15 columns for you, in your browser, with nothing uploaded. Three pages free, no account.

Frequently asked questions

Is this template the official AS9102 form?

No. AS9102 is published by SAE International and the forms are part of that standard. This is an independent spreadsheet layout for recording the same content. Check your customer flowdown and your quality system for any format they require.

Can I use this for AS9102 Rev B as well as Rev C?

The fifteen columns do not depend on the revision. The fields Form 3 must carry are substantially the same across Rev B and Rev C. Where they differ, the published form instructions for your revision govern.

Why is there no title block on the data sheet?

Because a title block above the header row breaks automated reading. Part number and revision live on the second worksheet instead.

Do I have to use all fifteen columns?

Item, Char. No., Dimension, Nominal, the two tolerance columns, Actual and Pass/Fail carry the meaning. The rest is traceability. If your flowdown requires fewer, drop them; if it requires more, add them at the end rather than in the middle.

Are you collecting email addresses for the download?

No. The links go straight to the file. There is no form, no account and no follow-up.
